These forms are also located in the New Practice Member Center section of our website so that you can save time in the office by filling out the forms prior to arriving. This paperwork provides us with your health history and information on your condition.

Our team will consult with you one-on-one in order to fully assess your health concerns. As a practice member, you are incredibly important to us. We want to provide for you the undivided attention you deserve so that we can best serve you to achieve your health goals.
ASSESSMENT
Our assessment, utilizing our state-of-the-art Core Score technology, will allow us to evaluate exactly how your nervous system is functioning. By truly understanding how your nervous system is functioning, we can provide you with the best possible care that you need and deserve.
CHIROPRACTIC POSTURAL X-RAYS
When appropriate, our on-site, digital x-ray suite allows us to view the exact structure of your spine. To see is to know, and to not see is to guess. We never guess with your health or our reputation.
FINDINGS
After thoroughly analyzing your assessments, we will review with you all of our findings, answer any questions you may have, and go over your recommendations for care so you truly understand the why behind your current health and the how behind you reaching your goals!
ADJUSTMENTS
One thing that makes us different is that each visit Dr. Agasar will perform a specific analysis to answer the following three questions: Do you need an adjustment today? If so, exactly where and how do you need to be adjusted? Did the adjustment work? By answering these questions each and every visit, we can ensure that you are receiving the best care possible and that you are progressing towards your health goals.
